The Medical Loss Ratio (MLR) is a metric used in the healthcare industry to measure the percentage of premium revenue that insurance companies spend on medical care and healthcare quality improvement activities, as opposed to administrative costs and profits. It is a key indicator of how efficiently an insurer operates and how much of the premium dollars are used for actual healthcare services.

Calculating MLR

To calculate the MLR, insurers divide the total amount spent on medical claims and quality improvement initiatives by the total premium revenue collected, expressed as a percentage. The MLR is typically reported annually and must meet certain minimum requirements set by regulatory agencies, such as the U.S. Department of Health and Human Services (HHS).

MLR Requirements

The Affordable Care Act (ACA) introduced MLR requirements to ensure that a significant portion of premium revenue goes towards healthcare services rather than administrative expenses or profits. Under the ACA, health insurance companies in the individual and small group markets must maintain an MLR of at least 80%, while insurers in the large group market must maintain an MLR of at least 85%. Insurers failing to meet these requirements may be required to issue rebates to policyholders.
